1. Please fill in your name, title, phone number, etc. in the spaces provided on the answer sheet.

2. There are 100 numbered questions. Indicate your answer to each one of them by making a mark in the oval (000) corresponding to the appropriate number on the answer sheet.
Move from left to right across the page.

+ (Plus) means I AGREE, YES or MOSTLY YES. M (Middle) means UNCERTAIN, MAYBE, neither YES nor NO. - (Minus) means I DISAGREE, NO or MOSTLY NO.
